Survey asking students to rate their anxiety regarding standardized science testing on a scale of 1 to 5, and to provide feedback and suggestions for the tutoring class.
Surveys will be conducted once every 5 weeks to monitor student anxiety levels.

  1. I think it is great that you are asking for students to give you their opinion about how the tutoring class should go. Even the small task of surveying them about their anxiety is showing that you care and getting them involved at the same time. If you haven't used Google Forms before, I suggest using it for your surveys. They automatically put all the results in graphs for you and I've found it pretty easy to use.
